I certify that the information provided by me for the purpose of employment is true and complete to the best of my knowledge.  I understand that if I am employed, any false statements or significant omissions will be considered cause for dismissal. Fat Shack is hereby authorized to conduct any verification of my personal, employment, education, credit, motor vehicle records and other related matters as may be necessary for an employment decision.  I hereby release employers, schools or persons from all liability in responding to inquiries in connection with my application. I understand that employment can be terminated at any time with or without cause, at the option of either the company or myself. I understand that no representative of Employer other than the President has the authority to enter into any agreement for employment for any specified period of time or to make any agreement contrary to the foregoing. I certify that I have read, understand and agree with the above.
